Check every order you want the court to issue. You can ask for more than one type. The more specific you are, the more clearly the court understands what you need.

Temporary Restraining Orders (§4–6)
Check every order you are asking for. A judge will decide at the hearing what to make permanent.
Stay away from (check all that apply):
Prohibited conduct — what you are asking the court to forbid (§4)
Check everything. The checked items will be assembled into the order description below, which you can then edit.
Child custody and visitation (§10) — if applicable
If you share minor children with the restrained person, you can ask for temporary custody and visitation orders here.
Child support and spousal support (§11–12) — if applicable
You can ask for temporary support orders at the same time as the restraining order.
Property orders (§13) — if applicable
Describe any property you need the court to protect — for example, your car, home, or other shared property.
